Getting ready for your dental implant surgery.
Dr. Porras will explain how to prepare for the surgical procedure, and give you options regarding to having the procedure done under local anesthesia or conscious sedation.
First, he has to examine your x-rays and CT scans if required to determine the bone quantity and the position of vital structures in your jaw bone (maxillary sinus, alveolar nerve, etc). In some cases he may indicate to perform diagnosis through computer guided technology for confection of a surgical guide (stent).
The surgery is usually performed under local anesthesia. This blocks completely the pain in your mouth and you will be awake during the procedure. You could decide to have conscious I.V. sedation, which will relieve anxiety and helps to relax while you are in a semi - conscious state, sedated but yet cooperative.
Dr. Porras will also discuss what will happen before, during and after the procedure. He will explain any possible complications and any pain that you may have. This is your opportunity to understand what is going to happen, and know other treatment alternatives.
This will help you to be informed and be able to give your consent for the procedure, which needs to be signed before the surgery.

What happens during and after the surgery?
Dr. Allan Porras and Dr. David Mendieta
Once the local has taken effect, Dr. Porras will perform an incision in the gums and then drill a small perforation into the bone. The implant will then be inserted in that perforation.
If you need to have tooth removed before the surgery, he can remove it and place the Implant in the alveolus of that tooth, this is known as Immediate Implant Placement. Other times he will leave the bone to heal for a few months, and this is called Conventional or Delayed Implant Placement.
Immediate or delayed dental implants can be done in one or two stages:
An artificial temporary crown could be placed the same day of the surgery (Immediate loading) if the problem is in the frontal teeth. But, usually we may leave the without the temporary crown for a few months until they heal (integrate). We can also place a provisional denture in the case we are removing all of your teeth.
After the implant is healed and integrated to the bone, you will have a second minor surgical procedure to discover the implant if its submerged. After this, we proceed to the second stage or placement of the final crown.

- The two stage treatment: the implant is placed in the jawbone and is left under the gums while it heals for a few months. Then a small incision is done to connect the post to the implant.
